For the first few dictations, it helps to write it all out so that you can get used to the flow of dictating without pulling stuff out your ass.

My first dictation was a 75 minute long, 10 page discharge summary.

I was lucky enough to have an intern there guiding me and cheering me on during my first dictation. Plus my hospital has this really cool computerized dictation system where you can rewind and even copy-paste parts of the finished dictate freely before you submit it. I think I listened the final version through like four or five times before submitting. :)
